A Texas dentist has been arrested more than three months after a 4-year-old girl died following a dental procedure, NBC reported.

Dr. Chrishelle Hemphill, 48, was arrested Tuesday, July 14, on a charge of injury to a child causing serious bodily injury, a second-degree felony, according to the Fort Worth Police Department.

The charge stems from the April 1 death of Aithana Rodriguez Arriaga, who became unresponsive after undergoing a frenectomy or tongue-tie procedure, at Cuddle Kids Dental Care in Fort Worth. According to an arrest warrant affidavit, the 44-pound girl died from meperidine, also known as Demerol, toxicity after receiving the opioid pain medication before the procedure.

The Tarrant County Medical Examiner determined the amount of the drug in her system was high enough to be considered lethal, and she was confirmed dead in the emergency room of Texas Health Harris Methodist Hospital Southwest Fort Worth, according to the case report online.

Investigators allege Hemphill told police she administered the medication to calm the child before the procedure but could not say how much had been given, according to the affidavit. Authorities also said the girl received three additional sedating medications along with nitrous oxide, which can suppress breathing.

The affidavit alleges medical experts found multiple failures in the child's monitoring and emergency treatment after she became unresponsive, including administering the wrong reversal medication.

Hemphill and members of her staff told investigators the child had undisclosed breathing problems, but the girl's physicians disputed that claim, telling detectives she had no history of breathing issues and was otherwise healthy. One of her doctors also questioned whether the child had a tongue-tie condition that required the procedure, according to the affidavit.

Fort Worth police said their Crimes Against Children Unit investigated the case and determined an offense had occurred, leading to Hemphill's arrest Tuesday afternoon.
